Living Your Life Beyond the Front Door

Being part of the community is about much more than “getting out of the house” – it is about belonging, contributing and feeling confident in everyday spaces.

Raha Community Care’s Participation in Community, Social and Civic Activities (NDIS Registration Group 0125) supports adults to take part in meaningful social, recreational and civic life in a way that feels safe, culturally respectful and genuinely enjoyable.

What Is NDIS Community, Social and Civic Participation?

Under the NDIS, Support Category 4 – Social, Community and Civic Participation – can fund a support worker to help you attend community, social, recreational and civic activities that are linked to your goals.

This may include joining a local group, going to events, learning new skills, taking part in hobbies or volunteering – with the right level of assistance so you can participate as independently as possible.

How Raha Community Care Can Support You

Our role is to walk alongside you – not in front of you. Together, we design supports that build your confidence, expand your choices and reflect what matters most to you.

Depending on your goals and NDIS plan, we can assist with:

Because Raha is nurse‑led and clinically informed, we can also coordinate with your other supports to make sure community participation remains safe and sustainable over time.

Examples of Activities We Can Support

Your activities should reflect your culture, interests, faith, personality and stage of life – not ours. Some examples of what NDIS Code 0125 can cover include:

Social and recreation
  • Going to a café, movie, local market or live event
  • Joining a hobby group such as art, music, gardening or gaming
  • Attending a gym, walking group or community fitness program
Community and civic participation
  • Volunteering with a local organisation or charity
  • Attending neighbourhood meetings, information sessions or forums
Skill‑building in the community
  • Practising using public transport with support
  • Building confidence to order food, ask for help or handle money
  • Learning how to plan a day out, check timetables and manage time
Faith, culture and family life
  • Attending religious services, cultural events or festivals
  • Staying connected with community groups that share your language or background
  • Supporting you to maintain important family and community roles

If you already have a goal in mind – or are not sure where to start – our team will help you brainstorm options and turn ideas into a realistic plan.

Our Difference: Nurse‑Guided, Multilingual and Local

Many providers offer “community access”; Raha Community Care adds depth through our nurse‑led, culturally responsive model and strong focus on Melbourne’s eastern suburbs.

Nurse‑guided, holistic support

Our registered nurses oversee overall wellbeing and liaise with your doctors and allied health providers where needed, so community activities align with your health needs and energy levels.

Multilingual and culturally aware team

We actively support participants from culturally and linguistically diverse (CALD) communities, offering staff who speak multiple languages and understand different cultural expectations around socialising, gender roles, faith and family involvement.

Eastern suburbs focus

Being locally based means we know the parks, centres, programs and services around Melbourne’s eastern suburbs, helping you discover activities that are close, accessible and welcoming.

Genuine relationship‑based matching

Wherever possible, we match you with support workers whose interests, culture or communication style complement your own, so time in the community feels natural rather than transactional.

Who Is This Service For?

Participation in Community, Social and Civic Activities (NDIS Code 0125) is suitable for adults aged 18 and over who:

We can also discuss privately funded community participation for people who are not currently accessing the NDIS.

How We Plan Your Community Supports

Your community participation journey with Raha usually follows a simple, collaborative process:

1. Listening and goal‑setting

We sit down with you (and your family or support coordinator if you wish) to understand your goals, interests, sensory preferences, cultural background and any worries you may have about being in the community.

2. Designing a personalised plan
Together we agree on the types of activities you would like to try, how often, and what kind of support worker will suit you best – including language, gender, personality and specific skills.
3. Risk, safety and accessibility planning
We consider things like mobility, transport, crowd tolerance, medication timing and emergency plans so that outings feel safe and predictable, not stressful.
4. Gradual, supported participation

We start at a pace that feels comfortable – this might be one‑to‑one support at quieter venues and then gradually building towards busier or more complex environments as your confidence grows.

5. Regular review and feedback

We check in frequently to see what is working, what is not and what you would like to try next, making adjustments so your supports continue to reflect your goals and interests.

Combining Code 0125 with Your Other Supports

Because Raha Community Care also provides nursing care, high‑intensity daily supports and assistance with personal activities, we can coordinate your community participation with the rest of your supports.

That might mean:

  • Aligning outing times with medication schedules or therapy appointments
  • Ensuring personal care is completed in a way that prepares you for your day in the community
  • Sharing information (with your consent) between support workers and nurses so everyone understands your preferences and safety considerations

This integrated approach helps you get the most out of your NDIS plan while avoiding duplicated or disjointed services.
